**This role is open to Care Coordinators currently working in Black Country Primary Care**

The Black Country Training Hub are seeking for expressions of interest for a Care Coordinator Ambassador. The post will be offered and funded on a sessional basis and will be equivalent to a Band 5 for one day per week on an ICS reimbursable hourly rate of £21.06 (including on-costs) until 31st March 2023. This may be extended after this period depending upon the success of the scheme. 

The Care Coordinator Ambassador will not be employed by the Black Country Training Hub, therefore if you are interested in this role you will need to seek approval from your Practice/Employer to undertake this commitment.

The Care Coordinator Ambassador role is to support the training and development of Care Coordinators across Black Country PCNs. The Care Coordinator Ambassador will provide holistic advice and guidance on the development of projects and will be a strong link to the Care Coordinator workforce to achieve the following outcomes:

·       To enhance and support the delivery of a planned Care Coordinators Network across the ICS;

·       To develop relationships with other Care Coordinators across the ICS to support integration and strengthen                        working relationships between Care Coordinators in PCNs to improve patient outcomes, better access, and                          support general practice workload;

·       To provide support to develop a Training Hub offer that supports PCNs with the recruitment of Care                                    Coordinators;

·       To provide Primary Care, Care Coordinators with support, advice and encouragement and promote the peer                        mentoring offer to increase uptake;

·       To support the development of a training needs analysis Care Coordinators in primary care;

·       To ensure that Primary Care, Care Coordinators feel valued as a key part of the Primary Care Workforce across

        the ICS;

·       To attend appropriate ICS meetings to provide the Care Coordinator voice in discussions;

·       To build relationships with Clinical Directors and other key stakeholders across the ICS;

·       To ensure that Care Coordinators working within PCNs do not feel isolated and have a voice in more strategic                      discussions.
