POST: ACP Pharmacist/Paramedic/ANP – Leading the Enhanced Health in Care Home service for the PCN
LOCATION: Health and Beyond Sites
REQUIREMENTS: Open to autonomous working ACPs working currently to Band 8a upwards
HOURS: 1 WTE - 37.5 hours per week
SALARY: 8b - £28.72 & £33.38ph
Job Purpose:
The role of the ACP within the Enhanced Health in Care Home team will work with a team to deliver the Framework of the service. They will lead a team of clinical staff alongside non clinical support and have an allocated GP attached to the service. They will line manage the clinical team and ensure that delivery of all aspects of care is provided as per the framework to the residents allocated within Care Homes to the PCN. They will work closely with care home providers to ensure high standards of care and support the wider organisation in delivery of all required KPI's aligned to Primary Care
Overview of Role:
· To fully understand the EHCH framework and expectations of delivery
· To have a full and robust line managment system with development for the clinical team
· To lead and support on all clinical aspects of care to the residents allocated
· To ensure a fully working system is in place for patient review and care schedules
· To deliver all aspects of KPI's within the scope of the framework and Primary Care
· To prepare reports and deliver to the PCN board with support from the Business Intelligence team
· To manage weekly MDT and outcomes
· To manage delivery of high quality care by working closely and supporting care home staff and management
· To clinically manage home rounds with the team, being the point of contact for all clinical enquiries and case management
