Area: Wolverhampton Health & Beyond – Ettinghsall Medical Centre

Essential:- Proficient use of the Emis and docman systems

Desirable, but not essential  - knowledge of clinical coding and summarising of medical records.  Full training will be provided for the right candidate.

The post holder is required to work within a team environment, supporting and working together to maintain workload.

The main responsibilities of the post-holder will be to review, action, summarise and read code relevant information from patient’s clinical letters into the practice computer system, working strictly in accordance with the practice policies, procedures and protocols. Ensuring data is added accurately.In addition to this the post holder will monitor and maintain the generic email inbox and distribute / action accordingly.

The post holder is also responsible for minute taking at meetings on a rotational basis.

Main duties/key tasks:

  • Code & file all incoming electronic post via docman & action in the same manner as paper post
  • Code all incoming post in regards to QOF and contact
  • Code or free text the clinical information on to the patient record
  • Deal with queries relating to coding
  • Identify relevant clinical information contained in patient correspondence
  • Reviewing medical records and producing an accurate summary of the patient’s medical history.
  • Read coding the information into the practice clinical system

Monitoring progress of notes summarising against practice targets

Monitoring patient call and recall systems 

  • Filing and retrieving paperwork
  • Follow practice protocols to ensure consistent and high-quality care is provided to our patients
  • Culling and sorting patient records and letters in preparation for summarising
  • Sort and date stamp all incoming patient-related mail. Scan patient-related documents onto their medical record using read codes as agreed by the clinical team
  • Add any additional information about the source of the document
  • Forward any actionable or informative the documents to the doctor or nurse who initiated the referral or who is most appropriate to receive the information
  • Ensure incoming letters are scanned onto the patient record within a 24/48-hour timescale
  • Scan all internal paper documents on to the relevant patients’ record and then store the documents in line with surgery procedures.
  • Code all incoming post in regards to QOF and contract
  • Redact third party information
  • Cover sickness/annual leave and work reasonable overtime when required, which may include some weekend hour
  • Maintaining, monitoring and actioning incoming emails in the generic email inbox
  • Minute taking for meetings on a rotational basis
  • Preparing protocols /flowcharts for procedures
  • Contacting docman/emis in relation to any issues as requested by Back Office Manager post holder
  • Maintaining spreadsheets
  • Training of all present and new staff on software systems
  • Actioning and monitoring of the generic email inbox
