This is a lunch and learn session for clinical and non-clinical primary care colleagues who are involved in the care of patients living with a serious mental illness (SMI) and would like to learn more about improving the physical health for their patients living with a SMI.
The session will take place on the 25th September via teams on 12:30pm 13:30pm.
The session will cover why physical health checks are so important and how we can better support patients with SMI to take up the physical health check offer to improve health outcomes. Top tips for undertaking the components of the check including using the nationally recognised Lester Tool and the importance of follow up interventions.
The session will also provide an overview of the service delivered by Black Country Healthcare Foundation Trust’s Physical Health Team and an opportunity for primary and secondary care colleagues to connect for the benefit of patient care.
