Diabetes UK and NHS Black Country ICS are running A Pro-active Approach to Diabetes Management and the 9 Care Processes Event on Thursday 15th June 2023, 12:00-15:30.
This will be a hybrid event. If you are unable to join the meeting face-to-face, you can join virtually from 12:30-13:30. Following that, there will be face-to-face workshops from 13:30-15:30 .
This pro-active approach to diabetes management is an information sharing session with interactive workshops to support practice staff in primary care, support people living with diabetes to achieve a better understanding to pro-actively achieve the 9 Care Processes for optimal diabetes management. Using an individualised practical approach to supporting achievement, this is the perfect forum to learn and apply skills and engage with health care professionals, all with a passion to optimise the care for people living with diabetes in the Black Country ICB.
There will be providers and stands that are independent to the workshop to support access to resources and information for practices under one roof!
