This event is open to all healthcare professionals in primary care and other sectors who are involved in the diagnosis and management of Heart Failure which includes GPs, Nurses (community, practice-based, hospital), Health Care Assistants, Pharmacists (community, ICB, practice-based, hospital, PCNs), social prescribers, health and wellbeing coaches, paramedics, physicians associates etc

Content

The content of the workshop will provide an update for primary care teams on aspects of the whole heart failure pathway, including case finding, diagnosis and treatment recommendations, and the national drivers that will enable PCNs to optimise care.

 

At the end of this workshop, attendees will:

• have an awareness of the recommended heart failure pathway of care.

• have an understanding of why heart failure is important in the wider context of Cardiovascular Disease prevention and in relation to CORE20Plus5.

• have an understanding of the novel therapies available for heart failure management

• understand the importance of case finding to support the identification of people who will benefit from heart failure therapies and/or optimisation.
